What you'll be doing
- Review cases identified as complex for identifying and promoting appropriate discharge planning. Assist medical director in acting as a liaison between department chairs, attending physicians, hospitalists, house staff, and physician assistants on issues related to documentation and discharge planning. Through daily review of census data, rounding on patient care units and interfacing with clinicians and other members of the interdisciplinary team, identifying system and process issues, throughput and discharge planning. Work collaboratively with CMRU and other relevant areas to develop and implement solutions. Serve as front-line triage and evaluator of complex cases. Collaborate with data analysis and reporting staff to identify and refine data needs. Collaborate with the medical director in the development of special projects/initiatives to address identified trends or problem areas in utilization and physician practice patterns.
What your background should be
- Related experience with discharge planning for complex and long stay cases. RN license and current registration. Administrative experience including directing and supervising clinical staff. Experience working with all members of an interdisciplinary team. Broad clinical knowledge base. Ability to analyze & present a variety of data to a variety of audiences. Experience with utilization, managed care and hospital finance issues.
