What you'll be doing

Provides paraprofessional nursing care within the residential hospice house or travels to patient homes to provide care within the established plan of care. Monitors the patients condition; answers call lights, anticipates patient and family needs and makes rounds on a regular basis. Takes vital signs. Takes and records temperature, pulse and respiration. Assists with patient positioning, patient ambulation, transfers, patient cleaning, toileting activities, catheter care, etc. Documents care in accordance with established procedures. Observes patient condition and records and reports changes in patients condition. Communicates changes in the patients condition, needs and any concerns to the registered nurse or hospice patient care manager. Aides with activities of daily living including assisting with bathing, dressing, toileting and grooming including shaving, skin care, hair care, nail care and oral hygiene needs. Changes bed linens. Interacts with patients, family members and others. Listens to patients and families questions and concerns and responds in a timely manner.

What your background should be

Must have two to three years of nursing assistant, home health aide or hospice aide experience is required. Must hold current certification as a nurse aide (CNA) in the State of Wisconsin. Must receive and maintain training as a hospice aide to provide hospice aide services. Must maintain a valid Wisconsin driving license. Must maintain proof of current auto insurance. Must have access to a vehicle in good working condition.

Required Schooling / Training

High school diploma.
