What you'll be doing

The clinical care coordinator is a registered nurse who collaborates with the hospitalist care team to coordinate and manage the process of patient care form the point of admission through discharge and transfer of care to the primary care provider. The person in this position works under general supervision, is responsible for various shifts, may be subject to over 40 hours per week and/or callback as required, and may also be required to remain on campus immediately before, during, and after severe weather and/or disasters.

What your background should be

Adult acute care experience required. Case management experience preferred. Utilization management experience preferred. Current RN licensure in the state of Florida BLS. Demonstrated proficiency in acute care nursing, knowledge and skills. Well-developed communication skills.

Required Schooling / Training

ASN degree or nursing diploma required; BSN preferred.
